The Community Cardiac Service supports people living with heart conditions to manage their health, recover after a cardiac event and stay well at home.
Support is tailored to your individual needs and may be provided in a range of ways.
Who can access the service
The Community Cardiac Service is for people who:
- have been diagnosed with a heart condition
- are recovering after a heart attack or cardiac procedure
- are living with a long‑term heart condition and need ongoing support
- would benefit from education, rehabilitation or monitoring in the community

- Recover after a heart event or procedure
- Manage your heart condition
- Understand your medications
- Improve your fitness and daily activity
- Make lifestyle changes such as diet, exercise and quitting smoking
- Manage stress, anxiety and low mood
- Reduce your risk of future hospital visits
How support is provided
Support may be provided in a range of ways, depending on your needs, including:
- Cardiac rehabilitation programs (exercise and education)
- Heart failure clinics and specialist nurse support
- Phone support and follow-up
- Home-based support
- Education sessions
- Referral to other services and support programs
Heart failure service
Heart failure support is available for people living with heart failure, cardiomyopathy or pulmonary hypertension.
Support focuses on helping you manage symptoms, understand your condition and stay well at home.
Cardiac rehabilitation
Cardiac rehabilitation supports people to recover after a heart attack, heart surgery or cardiac procedure. It includes supervised exercise, education and lifestyle support to help improve heart health and build confidence.
