Heart failure support helps people living with heart failure, cardiomyopathy or pulmonary hypertension manage their condition and stay well at home.
The service focuses on helping you understand your condition, manage symptoms and reduce the need for hospital visits.
Who the service is for
This service is for people who:
- Have been diagnosed with heart failure
- Have conditions such as cardiomyopathy or pulmonary hypertension
- Experience ongoing symptoms or frequent hospital admissions
- Need ongoing support to manage their condition at home
The care team will assess what support is right for you.
How this service can help
Support may include:
- Education about your condition and how to manage symptoms
- Help with medicines and treatment
- Phone support and follow-up
- Home visits when needed
- Clinic appointments and ongoing monitoring
- Support with exercise and activity
- Support for people with implanted cardiac devices
Support is provided in a range of ways depending on your needs, including phone, clinic and home-based care.
Heart failure rehabilitation program
A structured eight-week program is available as part of the heart failure service.
The program includes supervised exercise and education to help you improve your fitness, build confidence and better manage your condition.
Programs are delivered at Wyong and Woy Woy hospitals.
How to access the service
A referral is required to access this service.
Referrals can be made by:
- General practitioners (GPs)
- Hospital teams
- Specialists
