Chronic disease management

Chronic disease management involves ongoing care and support for individuals with long-term health conditions.

We provide a range of services to support people living with chronic and complex conditions, helping them manage their health, access appropriate services and stay as well as possible.

Support for people with complex chronic disease (Integrated Chronic Care)

Integrated Chronic Care supports people on the Central Coast who have complex chronic (long-term) disease and need extra help navigating their care.

Community Cardiac Service

Our Community Cardiac Service supports people living with heart conditions to manage their health, recover after a cardiac event and stay well in the community, including:

  • heart failure support
  • education and monitoring
  • cardiac rehabilitation

Community Respiratory Services

Community Respiratory Services support people living with long‑term lung and breathing conditions through education, rehabilitation and ongoing care in the community, including:

  • education and self‑management support
  • pulmonary rehabilitation
  • respiratory support for more complex care needs
  • support groups

Dementia Services

People experiencing memory changes or living with dementia and those who care for them, can access specialist support tailored to their needs.

Dementia Services provide assessment, support for changes in behaviour, care coordination and support for carers, including access to support groups.

Self-Management Support Service

The Self‑Management Support Service helps people living with long‑term health conditions build the skills and confidence to manage their health day‑to‑day.

Osteoporosis Refracture Prevention Service

The Osteoporosis Refracture Prevention Service helps people who have had a fracture related to weak bones (osteoporosis) to reduce the risk of future fractures. The service provides assessment, treatment and follow‑up care to improve bone health and prevent further injury.

Menopause Service

The Menopause Service supports people experiencing severe or complex menopause symptoms that affect their daily life. The service provides assessment, advice and care planning, working with GPs to help people manage symptoms and improve their wellbeing.
