Community nursing provides comprehensive nursing care that aims to keep clients in the comfort of their own home during periods of short or longer-term illness or acute disability. Community nursing services may be provided after a stay in hospital or following a referral from your GP. Services are provided at clinics in community health centres or the client’s home.

To access community nursing services, you’ll need a referral from your GP. GPs can send referrals by:

Services
Community nursing provides a range of services including comprehensive health assessment, wound management, palliative care, continence care and stomal therapy.
Services are provided seven days per week from 8 am to 4.30pm. Services may be limited on weekends and public holidays.
Through early recognition of illness and disease and prompt intervention community nurses work with clients to slow progress of the disease, minimise its effects and assist the client to achieve and maintain an optimum level of independence or a peaceful death when life can no longer be sustained.
