Adult Community Services

Some health conditions or chronic diseases can be treated in a community setting such as a clinic in a community health centre or an individual’s home.

You may be referred to an adult community service such as APAC or community nursing following a stay in hospital or a visit to your GP.

Commonwealth Home Support Programme (CHSP)

We provide community nursing and allied health services as part of the Commonwealth Home Support Programme (CHSP). CHSP services, along with many other aged care services, are subsidised by the Australian Government.

As a CHSP provider, we are required to work towards collecting client contributions for the services we provide.
