Megaphone image top tipsWe recommend submitting your completed documentation for processing as early as possible. All completed documentation must be submitted at least 6 weeks prior to your clinical placement.

Note: You will need to submit all completed documentation to your education provider who will submit ALL your documentation to the CCLHD Student Programs Team to update in ClinConnect.

All completed documentation must be combined and sent collectively as one PDF attachment. Only PDFs will be accepted. OneDrive, Dropbox, Google Drive, ZIP files, JPEGs, PNGs will not be assessed.

Your PDF file needs to be labelled “Compliance Documents – First Name, Last Name – Student ID

(e.g. Compliance Documents – Jane Doe – 123456)

Only colour copies will be accepted for assessment. Documentation must be in order of the checklist below.

Australian National Criminal Record Check (NCRC) (TVET School based students do not require a police check and students under the age of 18 do not require)

International Police Check – Stat Declaration for overseas students only.

NSW Health Code of Conduct Agreement for Students undertaking Clinical Placement

The Code of Conduct sets standards of ethical and professional conduct in NSW health. You must read the Code of Conduct (PDF) in full, print, complete and sign the NSW Health Code of Conduct Agreement for Students (PDF).

Vaccination Record Card for Health Care Workers

In order to protect yourself and your patients, you will need to be immunised against a range of specified diseases. Please read the policy directive:

Australian Immunisation Register (AIR) COVID-19 Vaccination certificate

All students are required to provide evidence of COVID-19 vaccination in accordance with the NSW Government Public Health (COVID 19 Vaccination of Health Care Workers) Order 2021, under the Public Health Act 2010. This is to ensure the safety of our people and our patients. All students attending clinical placement at any public or private hospital in NSW will be required to be fully vaccinated (two vaccinations) against COVID-19.

  • All digital certificates for COVID-19 vaccination are on the Australian Immunisation Register, available via myGOV
  • Sign in to your myGov account to get proof of your vaccination

This is the only proof that will be accepted for COVID-19 vaccination compliance.

NSW Health Appendix 7: Tuberculosis (TB) Assessment Tool

Tuberculosis (TB) Assessment Tool must be completed and signed by all students, including those who have never travelled overseas. Students requiring TB screening must commence screening before temporary compliance can be granted.

NSW Health Appendix 9: Hep B Declaration Form/ with attached serology report

Hepatitis B Vaccination Declaration to be completed and signed where a record of your Hep B vaccination cannot be reasonably obtained. Only an appropriately trained assessor such as a doctor, nurse or immuniser can witness the vaccination declaration.

Proof of Influenza Vaccine

According to NSW Health policy, influenza season is defined as running from 1 June to 30 September, unless another period is determined by the Chief Health Officer.

NSW Health accepts the following evidence for receiving FluVax:

  • Australian Immunisation Register (Vaccination Summary)
  • Seasonal Influenza Vaccine Consent Form signed by CCLHD staff health and wellbeing nurse
  • Patient Immunisation Summary/History (must be on doctor’s official letterhead)
  • Staff Health Immunisation Summary (NSW Health staff only)
  • Completed and signed Vaccination Record Card for Health Care Workers and Students
  • Record of evidence of influenza vaccination from a pharmacist

NSW Health Blood Borne Virus Student Declaration Form

If you are studying Medicine, Midwifery, Paramedicine, Dentistry or Oral Health, please read the NSW Health Blood Borne Virus Student Declaration Form (PDF) and sign and complete page 23. If you are not studying one of these professions, you are not required to submit this item.

The CCLHD Student Programs Team processes documentation compliance for multiple education providers therefore your documentation will be processed in the order of receipt. So do not delay completing your requirements for compliance.

In order to protect your privacy, you must remove the Individual Health Identifier (IHI) from your COVID-19 Digital Certificate and Medicare Immunisation History Statement. See the video below for more information.
