Getting started

To assist us in ensuring that you receive your first salary payment in a timely manner, please complete your new starter forms below and return to one of the options listed under completed forms before your commencement date. If you have any questions regarding the new starter forms please contact Recruitment on 4320 2522 (option 1).

Acceptance of employment

Your Letter of Offer and a copy of your position description are included in the email sent from Recruitment Services to you with the link to this site. Please ensure you read through both documents carefully and return a signed copy of each within 48 hours of receiving the email with this link.

Please complete all forms listed and return to CCLHD via the completed forms options below. To download these forms directly try Right Click (Control + Click for Mac) then choose the “save” or “Download” option in your browser menu.
