Aboriginal Youth Health Worker

​The Aboriginal Youth Health worker helps young Aboriginal people aged 12 to 24 years and their families through brief intervention, assessment and referral. The Aboriginal Youth Health Worker can link young Aboriginal people to local:

 

  • health services (such as Sexual Health, Drug & Alcohol, GPs and Mental Health)
  • accommodation services
  • education, employment and training services
  • sport and recreation services
  • legal services

The Aboriginal Youth Health worker is based at Wyong Central Community Health Centre but can provide a mobile outreach service to young Aboriginal people and their families by meeting in mutually convenient places.

Youth health worker on duty (intake)

​The Youth Health Worker on Duty service provides:

  • information and resources on health issues to young people, 12 to 24 years of age, and others who have concerns about young people's health and well-being
  • telephone consultation and brief intervention to young people, parents, carers and workers concerned about a young person's health and well-being
  • referral and triage to services, where available and appropriate to the information given
Priority is given to young people who make contact themselves, young homeless people and young Aboriginal people

The Worker on Duty provides information about relevant support on offer from the Youth Health Service, and information and advice about alternative services that may be appropriate to health concerns.

Youth Health Outreach Worker

The Youth Health Outreach worker helps young people 12 to 24 years who are homeless or at risk of homelessness through the provision of brief intervention, assessment and referral.

The Youth Health Outreach worker can link young homeless people to local:
  • health services (such as Sexual Health, Drug & Alcohol, GP's and Mental Health)
  • accommodation services
  • education, employment and training services
  • sport and recreation services
  • legal services

Based at Wyong Central Community Health Centre the Youth Health Outreach Worker can provide a mobile outreach service to young people by meeting in mutually convenient places.
